Be Prepared for Your Appointment

Please arrive 10 minutes prior to your appointment so that you will have adequate time to complete your new patient intake form. You may also download this paperwork (Patient-Intake-Form.pdf) and bring them in with you to your appointment.

Bring your current insurance card and co-payment, picture ID, and a list of your medical history, current medications, and any allergies that you may have. The name and address of any referring or primary physician with whom you would like us to communicate would also be helpful.

For existing patients, please be prepared to notify us of any changes to your insurance, address or phone number.

Understand your insurance benefits.

• Know whether your insurance requires a referral from your primary care physician.
• We are listed as a specialist for co-pays on most insurance plans.

We accept most major insurances including:

• Aetna
• Anthem
• Cigna
• Humana
• Medicare
• Medical Mutual
• United Healthcare

If your insurance carrier is not listed above, please contact your insurance carrier prior to scheduling to confirm that your visit will be covered.

If you wish to have a specific issue addressed such as a cosmetic concern, please let us know when you schedule.

Please remember to bring your identification, current insurance cards, co-payment, and lists of medical history, active medications and allergies with you to the appointment.

If you were referred by a physician or wish that our doctors communicate with your primary care physician, please bring the name & contact information of that doctor as well.

If you are a self-pay patient or if you are having a cosmetic procedure, payment is due in full at the time of service.

Facial skin is best examined in it’s clean, make-up free state. Please make arrangements to come to your appointment in that condition so that the doctor may see your skin in it’s most natural state. You may ask for facial cleanser at the time of the visit if necessary.

It may be appropriate for you to have a baseline full skin exam. This is often suggested for new patients or existing patients on an annual basis, especially if you have a history of moles on the body. Gowns will be provided to ensure your comfort.

Our office is centrally located off I-71 at the corner of Main Street and Washington Street. There is free parking in a flat lot directly behind the building between Cherry & Rich Streets. Please see our dedicated map area for directions on this website.
